Fillable forms for sample submission to the Plant and Insect Diagnostic Clinic (PIDC) are now available online. This will allow you to fill out the form online for printing or emailing.
The quality of your diagnosis is dependent on the quality of the sample itself and the information you provide to us on the submission form. If submitting a sample, please fill out as much information as possible on both pages. A detailed description, site history, chemical applications, and prevalence of the issues seen may be imperative for providing an accurate and timely diagnosis.
