Our hours of operation are from 8:00am-4:00pm. The doors to the ATRB building will lock at promptly at 4:00pm so try to arrive earlier. If for some reason you are going to be slightly later than 4:00pm please call us ahead of time at (515)-294-0581 and we can try to accommodate you. We are closed on weekends and University holidays

New forms

We are also proud to present our new submission form layout. We listened to our client's feedback providing more space to describe the problem on the front page by moving the table with questions on the back page. 

When looking for our form, please make sure to visit our website (clinic.ipm.iastate.edu), which will link to the extension store, the permanent location of the submission form. 

Our form is a living document that can be filled by typing on a pdf reader program, printed and included with your sample. Alternatively you can print the form and hand write the information.
