The ISU Plant & Insect Diagnostic Clinic provides diagnosis for plant problems and insect, plant or mushroom identification. Reaching the Clinic with your sample is now more streamlined, as a revised and simplified submission form to accompany your sample is now available. The “Sample Submission Form” (PIDC 0045) is now available through the ISU Extension Store and through the Clinic website for all samples except plant nematode analysis, for which the “Plant Nematode Sample Submission Form” (PIDC 0032) should be used. These two forms replace six forms that were previously available for submitting samples.
